Hair restoration isn't always about increasing the amount of hair on your head. Hairline lowering surgery makes the forehead smaller, making the frontal hairline more prominent.
It takes a refined aesthetic eye to capture the intricate balance between facial features and the hairline. If the distance between your hairline and eyebrows is too great, it can make you feel self-conscious wearing certain hairstyles or give the impression you are losing your hair. Hairline lowering at the Quatela Center improves facial proportions by reducing the distance between the frontal hairline and the brow area.
In addition to having expertise in hair restoration, our surgeons are board-certified in facial plastic surgery and have an in-depth understanding of the aesthetics of facial anatomy. They perform hairline lowering, also called forehead reduction surgery, to give the face balanced, natural, more attractive proportions. The beauty of choosing a facial plastic surgeon specializing in hair restoration is having access to a wide range of options. Dr. Quatela, Dr. Lee, and Dr. Montague take a comprehensive approach that includes surgical and nonsurgical options to accommodate different goals, lifestyles, and concerns.
Unlike hair transplant surgery, hairline lowering reduces the forehead, offering patients unique advantages, including:
Hairline lowering patients tend to be women; however, men also benefit from the procedure. The best candidates have the following attributes:
Our hair restoration surgeons will help you decide if forehead reduction is right for you during a private one-on-one consultation.
For a more pleasing transformation, our surgeons may recommend combining hairline lowering with other procedures, including:
Our surgeons perform hairline lowering as outpatient surgery using a trichophytic incision placed just inside the frontal hairline and another incision below it on the forehead. The trichophytic incision technique allows hair to regrow in front of the resulting scar. After making the incisions, the surgeon advances the existing hairline to sit lower on the forehead and closer to the eyebrows. After removing the excess forehead skin, the surgeon carefully sutures the incision closed.
Patients recover along the following general timeline (experiences vary):
Day of surgery: | Return home and rest. |
Day 3: | Tightness and soreness ease. |
1 week: | Sutures are removed. Resume normal hair-washing routine. |
10 to 14 days | Swelling and bruising fade. |
1 to 2 weeks: | Return to work. Incision healed. |
3 to 6 months: | Incision fully camouflaged by the hair. |
6 to 12 months: | Scalp numbness fades. |
For proper healing and optimal results, closely follow our pre- and post-operative instructions.

While the incision may seem extreme to some, it typically heals beautifully into a thin, flat scar well camouflaged within the frontal hair.
No, these are two distinct procedures. Hairline lowering moves the frontal hairline closer to the brows, reducing the area of the forehead. Scalp reduction removes balding areas from the upper scalp to address hair loss and reduce the number of follicles needed for a hair transplant.
While our surgeons use a meticulous technique when creating a trichophytic incision, there are rare cases where a hair transplant is needed to help camouflage the scar. We recommend waiting at least 6 months to assess hair growth in and around the surgical scar.
Our out-of-town patients need to stay in the Rochester area for one night after hairline lowering before flying or traveling long distances.
